Patient information: Full legal name, date of birth, and contact information
Provider information: Referring provider’s name, if referred by provider
Insurance information: Please upload your insurance ID card photos, front and back. Dr. Vasilev is "out-of-network" with all insurance. However, our patient advocacy team works with you wherever possible.
We do NOT accept MediCal or MediCare, but supplemental policies may apply (not Advantage Plans).
